I remember once trying to purchase something from an online vendor that I eventually ended up not getting because on their order page there was no way for me not to choose an honorific when inputting shipping information.
I have a name. I am not a title.
I feel in no fashion disrespected when someone uses my name instead of referring to me as something that has nothing to do with me. In a way, I almost feel not disrespected, more, well it frustrates me. My last name is not my own. A last name is usually something that belongs to the father or the husband. A first name belongs to you (even if you didn't choose it. You can make it yours.)
Addendum: I was at the bank today, you see, getting a roll of loonies, and had to talk the teller around to referring to me by my first name rather than Mrs. Fraser. I am not married. Mrs. Fraser is my dead grandmother - I am Lonita.
